Madhya Pradesh Society for Promotion of Information Technology (MAP_IT) on behalf of the Department of Information Technology, Government of Madhya Pradesh invites nominations for Awards for Excellence in e-Governance initiatives in the State of M.P. The application against each nomination must comprise the following:

• Nomination form is to be submitted in the prescribed format and completed in all respect.
• It is mandatory to submit the form ONLINE along with hard copy as specified below to MAP_IT office.
• Before submitting the form online you need to register yourself and create an account in the login pannel.
• Each Nomination should also be submitted in sealed envelope with clearly indicating on top, the category of Award for which the application is being submitted along with the USER ID and REGISTRATION ID created by you in online form submission. If an individual or organization is submitting Nomination in two or more category then each submission should be made separately.
• A write up with all necessary details as per guidelines laid down in the selection criteria for each award should be attached (Both for Online Submission [as MSWord Doc] and Hard Copy).
• The Nomination form to be submitted in three signed copies to reach the following address by the date mentioned in the advertisement.
